format and requires the Adobe Acrobat Reader to use. If you do not all
ready have it, you can download Adobe Acrobat Reader for free at the site
below and then use it to print the form or you can complete the form
on line and e-mail it to the Membership Chair directly by selecting
the red and yellow submit button at the bottom of the form (we'll still
need your cheque though!)

with your name, complete mailing address, home, fax, and business telephone
numbers, e-mail address, occupation, years on staff at HSR, positions held
and any other personal information which you would like to share (weddings,
births, graduations, promotions, trips, awards received, etc.) and mail
